Any patients wanting to register with our practice can do so if they are within a 6 mile radius. Please read the information below to ensure you are entitled to free healthcare in Northern Ireland

Within Northern Ireland:
If you are registering within Northern Ireland you must fill in two forms below.
New Patient Health Questionnaire
HS200 application form
The forms can be emailed although the patient must come into the practice to sign the forms and bring identification. i.e. passport.
Alternatively they can be left into the practice.
Entering from outside Northern Ireland :
If you are registering from another country you have to fill in the two forms below.

The forms can be emailed, although patients must come into the practice to sign the forms and bring a form of identifiacation required. i.e. passport
Alternatively the forms can be left into the practice.
Patient previously registered in Republic of Ireland need proof of current address in Northern Ireland.
