Any patients wanting to register with our practice can do so if they are within a 6-mile radius. Please read the information below to ensure you are entitled to free healthcare in Northern Ireland.

Within Northern Ireland:

If you are registering within Northern Ireland, you must fill in the two forms below:

The forms should be signed and brought to the reception desk. You should also bring photographic identification and proof of address. If you wish to email the completed forms back to us, you will need to report to reception to sign them and provide us with the above documentation.

Entering from outside Northern Ireland but within the UK:

If you are registering from another country, you must fill in the two forms below:

The forms should be signed and brought to the reception desk. You should also bring photographic identification and proof of address. If you wish to email the completed forms back to us, you will need to report to reception to sign them and provide us with the above documentation.

Entering from outside the UK:

If you are registering from another country, you must fill in the two forms below:

The forms should be signed and brought to the reception desk. You should also bring photographic identification, proof of address, and proof of lawfulness to reside in Northern Ireland from the Home Office. If you wish to email the completed forms back to us, you will need to report to reception to sign them and provide us with the above documentation.

Patients previously registered in the Republic of Ireland need proof of current address in Northern Ireland.
